To watch these videos you must be a School Member  - Your monthly tuition covers access to the learning database and there are videos for all members to watch and the specific curriculum members only you can see for your martial art style and belt level.  All members should have received a email with registration information. To access these videos you must register with your email, if you did not receive a registration email check your junk mail or call the academy 615-989-7945 Ext 11 to verify your email on file.

Non-members can purchase the online access for $60 per person per month or $500 discounted yearly. Call to set-up this service and the 12 month subscription. WAIVER & RELEASE FORM

You agree t release GMA -Gallatin Martial Arts Center In / Global Martial Arts USA from all claims of injury, death or harm. Because physical exercise can be strenuous and subject to risk of serious injury or death we urge you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity. You agree that by participating in physical exercise, training activities or watching any of these videos, you do so entirely at your own risk. Any recommendation for child development or changes in diet including the use of food supplements, weight reduction and/or body building enhancement products are entirely your responsibility and you should consult a physician / doctor prior to undergoing any dietary or food supplement changes. 
 
You agree that you are voluntarily participating in these activities and use and rental of these training videos / developmental videos are viewed at your own accord and you assume all risks of injury, illness, or death that may be caused by these martial art videos, exercises videos, training techniques videos, self defense techniques videos, weapons training videos or child development videos. 
 
GMA and its teachers, Master, instructors, coaches and staff highly recommend that all training by minors should be supervised by parents or guardians during training periods and any online activities to avoid any serious issues or injuries. You as a parent of a minor assume all risks, responsibility and liabilities and understand that GMA and it staff do not pay for or cover any injuries or liabilities. If you decide to train with a partner you assume all legal ramifications and responsibility for your training partner/ partners and you are solely  responsible for anyone that is viewing or using these video. These videos are only for your use and are being rented to the member/buyer as long as the paid membership is active. As a member or buyer be aware that these videos are copyrighted and trademarked and all videos may not be reproduced or copied in any manner and all video’s are protected by copyrighted laws.

GMA and it staff are also not responsible for any loss of your personal property or damages of any kind during training. You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability. You expressly agree to release and discharge / indemnify GMA the masters, trainers or instructors from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the the school, owners, masters, trainers, coaches or instructor for any personal injury, death or property damage. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from. By paying for the service or by clicking the student portal button you agree to this release, I acknowledge that I understand its content and that this release cannot be modified orally.
