GMA Martial Arts - Medical / Release of  Liability Waiver

By signing this electronic waiver:

 You the parent / guardian or buyer, fully understand as a member parent or non-member parent that your child shall be engaging in physical activities, including but not limited to, training and instruction in the Martial Arts, which necessarily includes physical activity and contact which could cause injury to you (The “Activities, Parties, Nerf Wars, Games, Bouncers, and any Martial Arts Activities). You are voluntarily participating in the Activities and assume all risks of injury, both physical and mental, which might result.  You hereby agree to waive any claims or rights you might otherwise have against the the academy / school (GMA)Gallatin Martial Arts Inc, (DBA) GMA Martial Arts, Global Martial Arts USA, its affiliates, its organizations, its unions, its owners, its officers, its instructors, employees, agents and assigns (collectively the “Releases”) for injury, loss or damage or death arising out of or relating to the Activities including, but not limited to, those risks which may be associated with or attributed to any negligent act, omission or fault of the releases  from and against any claim, loss or damages, including but not limited to their attorney’s fees, court costs, and loss of income, to which they may be subjected arising out of, or relating to the training activities including, but not necessarily limited to, the “Waived Risks”.  

You further agree to release (GMA) GMA Martial Arts Center, Gallatin Martial Arts Center Inc, GMA Rocian Gracie Brazilian Jiu-Jitsu / GMA MMA  (all “Releases) from any liability for any loss or theft of personal property.  

Events / Birthday Parties

The terms of this waiver shall survive the termination/expiration of the Membership Plan and shall continue in full force and effect with respect to any and all post Membership Plan use of the martial arts facility by member, or other post-membership plan affiliation of Member and Releases. I as the authorizing parent that scheduled a birthday party or registered any child for the Nerf War Night / Parents Night Out  assume any medical cost arising from the event and understand that all participating must have medical insurance to cover any injuries. You also understand all attending any event must fill out the GMA online activity waiver prior to event by a legal parent or guardian over 18 years of age. Member/Parent/Guardian warrant, represent, and agree that attending guests are in good physical condition and that he/she has no physical disability, impairment, or ailment  preventing him/her from engaging in active or passive exercise or that will be detrimental or inimical to his/her health, safety, comfort or physical condition or that of others. 

GMA is a traditional martial arts schools:

Only GMA Equipment / GMA Gloves / GMA MMA Gear / GMA Shirts / GMA Shorts / GMA Rash Guards and GMA Uniforms are permitted on the mats.  All our equipment is approved of better safety and sold at a discounted rate. To help us maintain a professional school and a positive family team environment.  If you are on the mats during classes or open gym times, you must wear GMA gear and clothing. Students not wearing proper gear will need to sit out of class. (Please Review Online Student agreement) Proper GMA attire helps our staff identify members vs people just non members and assets in keeping all members safe. Please note these are our school requirements as many martial arts schools have.  If you are looking for more of a lax environment towards discipline, manners, values there are many other academies that are non traditional.  In the end martial art means military way. Our end goal is to develop family and respect for the process of learning and inner growth. 

Weight Room: Adults 16 years of age or older, may use our training room at no cost if they are martial arts members or parents.  However if the training you must follow the gym requirements and go through the rules with one of our staff members. Safety is the key and cleanliness so use a GMA towel or bring your own towel to wipe off the sweat and you must wear workout clothing and sneakers. No sleeveless shirts or tank tops (Rules are posted) No Children in the weight room. All children should be monitored and should not run around the facility.
